Preparing for Your First Teletherapy Appointment

Below, we’ve outlined step-by-step instructions you’ll need to take leading up to your first appointment and what to bring to your first telehealth session. This will help us provide quality behavioral health services, assure seamless payment by your insurance provider and help make you as comfortable and prepared as possible during your first appointment.

1.     Schedule Your Appointment by calling 866.852.4001.

2.     Complete the Pre-appointment questionnaires.

At Least 24 Hours Before Your Appointment.

In order to provide the highest quality service, we need you to:

  • Share the concerns you would like addressed, your personal and family history, as well as any medical conditions or current medications
  • Sign a consent to treatment
  • Choose your method of payment for any co-payments and deductibles for your session

Gather your information together before you start. The online form is quite comprehensive and could take approximately 30 minutes to complete.

To start your pre-appointment forms:
  1. Go to MyChart sign up page.
  2. Get your MyChart Activation Code from the email you were sent. If you can’t find your email or it’s been more than 2 weeks since you received the email, please call us to get a new activation code.
  3. Once you’ve activated your MyChart account, complete the forms waiting for you.

When you called in to schedule your appointment, our staff verified that your insurance covers telehealth outpatient behavioral health services. However, a preauthorization may be required by your provider. Please contact your insurance company to verify benefits and secure any preauthorization requirements. If a required authorization is not obtained, you will be responsible for payment of services.

5.     Find a Private Location.

For your first and all following appointments.

Choose an appropriate location, room or setting for your appointment where you can talk freely without being overheard or interrupted. We will be sure to do the same on our end.

Please plan to log in 10 minutes prior to your appointment start time.

6.     Connect with Your Therapist.
