How can I sign up for MyChart?

For Computers and Laptops

Sign up for MyChart on your computer here.

For Smartphones and Tablets

Download and install the MyChart app to your phone or tablet and search for Pine Rest.

• Download MyChart app for Apple
• Download MyChart app for Google

You’ll be asked whether you have an activation code.

• If your care provider gave you one, click on “I already have an activation code.”
• If your provider did not give you a code, or if the code expired, just click on “I do not have an activation code.”

Then follow the prompts to finish setting up your account.
