Care Everywhere Referrals
Pine Rest uses Epic (referred to by patients as MyChart) for our outpatient, inpatient and residential Electronic Health Records (EHR) system. Care Everywhere allows healthcare providers with the Epic EHR to securely access and update their patient records with information from other healthcare organizations, facilitating information exchange and coordinated care, even across different Epic systems.
Care Everywhere will allow you to…
- Refer a patient to any of Pine Rest:
- Outpatient services including psychiatry, psychotherapy, teletherapy, testing and assessment, addiction services (all levels), ECT and TMS
- Inpatient Services
- Partial Hospitalization / Day Programs
- Search for patient info including automated query-based exchange, closed-loop referrals management and patient-initiated record sharing.
- Directly message our providers and obtain documents, such as Summary of Care Documents
- Reconcile Medications, Allergies and Problems List
Care Everywhere Benefits
Schedule patients faster. Referrals sent to Pine Rest using Care Everywhere are automatically created in our work queue and scheduled quickly. It also gives us the ability to send referral letters to notify you that the referral occurred.
Improved care coordination. Information about referrals between our organizations will be synced, so updates on status, number of visits and scheduled appointments can be seen in real time.
Informed patients. Referrals appear in MyChart, so patients can track their referrals in one place.
Enhanced patient outcomes. Access to timely and accurate information can lead to faster diagnoses and more effective treatments.
