Patient health information includes any information included on your personal electronic health record. The information that will be exchanged may include information about your:
- Allergies
- Medications
- Medical, surgical, family and social history
- Immunizations
- Clinic and hospital visits
- Lab and imaging test results
- Diagnoses
Care Everywhere can only be used during a visit to facilitate your care which improves the quality of service that your clinical team is providing you. This is called “Continuity of Care.” Care Everywhere cannot be used to request your records “just to have.” All requests are monitored and can be audited to ensure your records are being used to benefit you as the patient.
Yes. Medical records are encrypted and sent directly and securely to the requester, which is much faster and safer than fax or mail. We follow all federal and state laws about protected health information, and all requests to share a patient’s medical record are recorded.
No, there is no fee to the patient for this service. Care Everywhere is a free service provided to help improve the quality of care that you are receiving here at Pine Rest Christian Mental Health Services and also at other organizations.

Additionally, Pine Rest can share with organizations that are using an HISP (Health Information Systems Program). In Michigan, GLHC (Great Lakes Health Connect) is the HISP connecting private practice offices with other healthcare organizations.
