Pine Rest’s Psychiatric Home Health Care provides individuals with unique behavioral health needs with quality, in-home services, which are covered by the Medicare home care benefit. A registered nurse, certified in mental health, works with the client, family, and physician to develop an individualized treatment plan. The goal is to maintain the client’s independence and avoid re-hospitalization.

• 24-hour access to an on-call psychiatric nurse.
• Support in home and assisted living settings.
• Coordination with medical nurses, social workers, chaplains, dieticians, and physical, occupational, and speech therapists as needed.
• Regular assessments of safety of home environment, general physical health, medication adherence, monitoring side effects and benefits.
• Affiliated with HomeCare of Holland Home and Saint Mary's Home Care.
• Frequency of visits is usually 1-3 times a week.

Eligibility requires patient is homebound and under the care of a physician.
